2018-2019 Registration Forms

Registration 1 (all the forms that must be returned by May 25th). 
  • Tuition contract
  • Emergency contact- one form per student
  • Code of conduct
  • Photo Release
  • School survey
  • Extended care- if needed, one form per student. Registration payment of $25 must be submitted to the office.
All forms are in fillable pdf format. You can either fill out the forms and return them to LaChon Richardson via email (l.richardson@stfinnbarr.org) or print them and send them to the office.

If you prefer a printed copy of the registration packet, please let LaChon Richardson know and one will be sent home with your child.
 
NOTE: Students that return all forms by the due date will have free dress Friday, May 25th. ☘
