Registration Paperwork

The attached documents make up our school registration packet for the 2017-2018 school year. 

 

Registration forms that must be returned by May 26th are:

  •           Tuition Contract
  •           Emergency Contact - one form per student
  •           Code of Conduct
  •           Photo Release
  •           School Survey
  •           Extended Care - if needed, one form per student. Registration payment of $25 must be submitted to the office.

 

All forms are in fillable pdf format. You can either fill out the forms and return them via email (l.richardson@stfinnbarr.org) or print them and send them to the office.

If you prefer a printed copy of the registration packet, please let LaChon Richardson know and one will be sent home with your child.

 

NOTE: Students that return all forms by the due date will have

free dress Friday, May 26th.
